Blood Ties
Go to the Nilfgaardian Army Camp south-east of Velen and approach the wailing woman. Offer to help her track down her son. Go south towards the biggest building, then go inside and speak to the quartermaster. Bribe him or use Axii to force him to reveal the location of the missing soldier, then head to the swamp to the north-west.
Kill the ghouls, and use your Witcher Senses to follow all of the trails nearby. Eventually you'll see the red-headed man hanging from the gallows. Examine him, then take the letter to his mother. Choose your response to finish the quest.
Deadly Crossing (3)
Go to Dragonslayer's Grotto, and look west towards a wooden bridge.
Hazardous Goods
Eventually you'll bump into Gert Borel, a humble man with a cart who's being harassed by ghouls. Kill them, them speak to Gert about the bodies on his wagon. Insist on torching the corpses so they can't spread disease, and use your Igni Sign to do exactly that.
You'll find Gert in the battlefields of Velen by Reardon Manor later on. Kill him if you wish to put him out of his misery.
Looters (3)
You'll find the next bunch of blighters by Toderas. Kill all seven of the infantrymen to finish up the quest.

Follow the quest marker, talk to the sage, and then head for the cave. Kill the ghouls, pick the dragonsroot, return to the old man, and listen to his prophecy. Ta da!
